Out of Town Order Form

This order form was designed to accommodate customers who are unable to be measured by a CK Bridal Consultant. CK Bridal Boutique will not place the order until all attendants have provided their measurements and we have received a 50% deposit.  A rush fee may be applied if wear date is within 4 months or less of wear date (wedding date).

*NOTE: If you are taller than 5'8" you may need extra length or if your measurements suggest a size larger than a 16, additional charges will apply and vary with each manufacturer.

*NOTE: A CK Bridal Consultant will contact you to ensure we make every attempt to order the correct size according to the measurements provided.  Designers DO NOT offer standard sizing.  Most designer gowns run smaller than normal clothing.  Your measurements along with the designer size chart are used to determine the size of gown to order.  Alterations are usually necessary to ensure a custom fit; alteration charges are not included in the cost of the gown.

Delivery Option

We will ship your merchandise to you for an additional charge of $20.00 (shipping, handling & insurance).  All merchandise and freight must be paid in full before shipping.
